1. Package Validity and Usage:

 

The VIP Neuropathy Treatment Package voucher must be utilized within 30 days from the date of purchase. The package holder is required to schedule and attend their initial appointment within 30 days of purchase.

 

 

2. Cancellation and Rescheduling:

 

The applicant is allowed a maximum of two (2) cancellations or rescheduling requests for their appointment. These requests must be made at least 24 hours in advance. Failure to provide a minimum of 24-hour notice for cancellation or rescheduling, or exceeding the maximum limit of two (2) rescheduling requests, will result in forfeiture of the special offer.

 

 

3. Payment Policy:

 

Payment for the VIP Neuropathy Treatment Package is non-refundable. Acceptance into our neuropathy program is not guaranteed solely based on payment. A thorough evaluation and assessment will determine eligibility.
